| Medical Certificates |
|
If you think you may need a certificate for work, school assignments, or other study to explain why you were absent, or unable to complete your assignment on time, you need to have seen a doctor. The certificate supplied is a legal document and cannot be provided unless the doctor who signs it has seen you.
For similar reasons, if you need renewal of a sickness benefit or a Disability Allowance, or another subsidy based on health needs, you need to see the doctor who is going to sign this form. If the expense seems excessive, you can apply to WINZ for a Disability Allowance to cover medical costs.

| Divers |
| Recreational, sports divers

This includes all divers who want to complete a PADI course. We book a standard 15 minute appointment for this.

| Commercial (Dr Rex Browne only) |
|
We book a 3/4 hr appointment and ask you to arrive 30 mins early to complete paper-work and some measurements with the nurse. You need to bring your log book, and a cheque for $50 to send with the medical, for registration. You should check if you need to have your audio exam done before being seen here - RNZNH, Bay Audiology, or Hearing Association can do this.

| Driving |
| The LTSA requires a periodic medical exam for drivers with passenger service and vaious heavy vehicle classes of licence, and for all private drivers after their 75th birthday. |

| Immigration |
|
The NZ Immigration Service will explain what they want you to do. Follow their instructions. For the medical, you usually have 15 minutes with the nurse, a 30 minute appointment with the doctor, some blood tests, and have to go and get a chest xray, paying as you go. It expires after 3 months and its a hassle for you to try and convince us we should fill the whole thing out again for free.

| Insurance |
If you're applying for insurance, the company often just requires a medical report, maybe some blood tests, and asks you to sign to consent that it's ok to contact us. Sometimes they require a medical and they may ask that this be done by a specially trained nurse, a contracted doctor, or that you see your usual doctor. The company pays.
Don't hide anything. If you ever saw the doctor about a problem, declare it, because they do investigate claims and refuse to pay out if there's any evidence of dishonesty. We've seen this happen.

| Pilots (Dr Alastair Borwick only) |
We keep the relevant CAA forms in the office, or they can be printed off the CAA website (go to Medicals / Forms for Medical Certification / open and print off a 24067-001 form.

- 45 minutes appointment with the doctor
- if AB has not met you before, you must bring in a form of ID such as a passport, or a drivers licence with photograph
- if you wear contact lenses, these need to be left out overnight so that uncorrected vision can be tested
- bring your current pilot licence or certificate if you hold one
- we book you to arrive 30 mins prior for paper-work and some nurse measurements.
- cost - $260 for first certificates, or $230 for repeats.
