Funding changes and subsidies that have been introduced in Primary Health over the past 3 to 4 years require people to be enrolled at only one medical practice at a time, in order to get a subsidy. This requires that you sign an enrolment form. The front desk staff manage these enrolment forms, and legally they have to be sure that they are enrolling NZ citizen or an eligible person, so may ask to see proof of citizenship, or eligibility if there is any doubt.
You can print an Enrolment form (214kb) and complete it before you come to see us for the first time.
