29 Anne St, Devonport, Auckland NZ. ph 9 445 3483 (24 hours/7 days) Office email We want our website to make it easier for you to use our GP services at the Anne Street Medical Centre in Devonport, so it tells you what we do and how we do it. We hope this is useful to people new to Devonport, or even NZ, as well as to our current patients.

SATURDAY MORNING CLINICS

new hours - 9:30AM - 12:30pm

From May 2011, we're starting the Saturday morning clinic 30 minutes later at 9:30am, so staff can watch some of their childrens Saturday sports.
No appointments, a walk-in service staffed by experienced local GP's.
We want to provide an alternative to that trip down Lake Road for non-routine, or urgent medical problems.
(please note, we cannot do regular health-checks on Saturday mornings)

To qualify for subsidies and reduced fees, people can only be enrolled at only one medical practice at a time. This requires that you sign an enrolment form. The front desk staff manage these enrolment forms, and legally they have to be sure that they are enrolling NZ citizen or an eligible person, so may ask to see proof of citizenship, or eligibility if there is any doubt.

You can print an Enrolment form (214kb) and complete it before you come to see us for the first time.
Or if you have not been to see us for 3 years, you can print a re-enrolment form , and send it, or bring it, in to us.

Questions, suggestions or comments are welcome.
